Your first visit, without the guesswork
Here is what to expect when you join the practice — from requesting a visit to walking out with a plan.

Request a visit
Use the appointment form or call the office. Tell us whether you are a new patient and what you hope to address first.
- Step 2
Share your history
We will ask about medications, pharmacies, prior records, and any specialists you already see. Bring what you have — we can help fill gaps.
- Step 3
Come a little early
Plan to arrive 15 minutes before your first visit so we can complete registration without cutting into your appointment time.
- Step 4
Leave with a plan
You should go home knowing the plan, how to reach us, and what happens next — labs, follow-up, or simply when to return.
What to bring
- Photo ID and insurance card, if you have one
- A current medication list, including supplements
- Recent records or a list of other clinicians, if available
- Your questions — written down is helpful
Insurance and self-pay
Insurance panel participation and self-pay rates are being finalized. Please call the office to ask whether we can see you under your plan and what to expect if you pay directly.
Accepted insurance plans and self-pay rates will be posted here as soon as they are finalized. Until then, the office can talk through options by phone.
