What this page is — and is not
This is a short privacy notice for the public website. It is not a HIPAA Notice of Privacy Practices and it does not describe how medical records are used or disclosed in the clinic. A formal Notice of Privacy Practices can be added or linked here when the practice is ready.
Information you send us
If you use the appointment request form, you may share your name, phone number, email address, a preferred time, and a brief reason for the visit. We use that information only to respond and to help schedule care.
If the form is still using the default mailto option, your message is prepared in your own email app and sent to Advancedhealthyoptions@gmail.com. If a form service is later connected, that service will also receive the details you submit.
What we do not ask for here
Please do not send Social Security numbers, payment card details, or detailed medical records through this website. Those belong in a secure clinical process, not a public form.

This site is built as a simple marketing website. It does not currently embed advertising or third-party analytics. If that changes, this page should be updated.
